Introduction to Transition Planning 2.0

Learn what transition planning is, how best to prepare for it, and key roles involved in the process. You will also identify your support network to help as you step into postsecondary life. Additional information on resources and agencies will be provided. This workshop targets young people with disabilities, ages 14-26, but parents/caregivers and professionals are also welcome.
